Debuting updated data and tools for health and equity
The 2025 Annual Data Release debuts a bold new way of broadening our understanding of health along with updated data and tools. The resources are designed to help improve health and equity, so everyone has what they need to live long and live well.
A bold new model of health
We know that many conditions — like income, safe housing and education — shape how long and how well people live. For over a decade, County Health Rankings & Roadmaps has measured these conditions through data and our model of health. But how are these conditions created? And who creates them?
The University of Wisconsin Population Health Institute (UWPHI) Model of Health provides a framework to understand how societal rules and power shape these conditions. Explore the new model here.
Report focuses on ways to build power and change the structures that impact health
The 2025 report uncovers and challenges the structures and forces that create community conditions, and in turn shape health and well-being. It introduces the core concepts behind the UWPHI Model of Health and looks at the impacts of power, governance, structural racism and societal rules on community conditions like affordable housing, income and education.
Updated data
We have updated our collection of over 80 health-related measures with the most recent data. For nearly every county in the U.S., we provide a data snapshot that includes refreshed trend graphs, and updated analysis of what’s working well and areas for improvement. New measures this year include access to parks, lack of social and emotional support, feelings of loneliness and adverse climate events.
You can also explore a new national map feature that shows regional patterns and provides comparisons of a county’s health with other counties across the nation.
Healthy communities require everyone to work together. Search for your state or county at our Health Data section to learn more about what influences how long and how well people live.
What Works for Health strategies, curated list and AI tool added
We’ve made three updates to What Works for Health (WWFH), our database of over 400 evidence-informed strategies to help create thriving communities:
- Learn about Community organizing in public health in our newest addition to WWFH.
- Find a strategy to address structural determinants of health in your community by using our new curated strategies list. It includes 40 solutions that address power and societal rules, including laws, policies, budgets and governance.
- Navigate the entire database using Evi, an AI tool designed to help users find information. Use Evi to discover solutions that best fit your community’s needs.
